Create a maker-checker approver group

/maker-checker-approver-groups

Creates a new approval group for the authenticated entity so maker-checker workflows can be routed to a defined set of approvers. The service derives the entity_id from the caller's active session and validates that the group name is unique for the same entity and workflow type. Each approver must be a valid identity, and duplicate approver identities or approval order numbers are rejected. When is_default is set to true, the new group becomes the default active group for that entity and type, and any existing default group is automatically demoted.
